At Eye on Evanston, access to experienced eye care is important to us. We work with several insurance providers to make sure you get the treatments and care you need.

We’re happy to submit to the following insurance companies on your behalf. Please note that some plans will require that the policy holder submit the claim or they will only reimburse directly to the policy holder.

Alberta Health Care Coverage

Alberta Health will cover all eye examinations for patients ages 18 and under and 65 and over, including routine comprehensive annual exams.

For ages 19-64, routine comprehensive eye examinations are not covered under Alberta Health, but medically necessary office visits are.

Alberta Health provides coverage for medically necessary eye problems such as:

Eye infections, scratches, red or pink eye, pain or sudden vision loss
Foreign body injury
Retinal examination and diagnostic imaging for diabetic patients
Monitoring and treatment for chronic eye diseases such as glaucoma, macular degeneration, and cataracts
Pre and postoperative surgical care

Routine eye care including eyeglass prescriptions and all contact lens fittings for ages 19-64 may be covered through 3rd party insurance. You may be eligible for coverage of these services under your personal or employer health care plan.
